The Treatment Outcome Package (TOP) is a questionnaire that is filled out by patients before, during and after treatment. There are different questionnaires for children, adolescents and adults.
Quick and Easy.
The TOP is written at a fifth grade reading level and takes an average of 8 minutes to complete. For adults and adolescents, there are 58 questions. For children, there are 48 questions.
With TOP, patients are more likely to disclose information that may be too stressful to share in conversation. TOP provides an efficient way for patients to share their full story. TOP is also an effective means to show patients their progress during treatment.
Paper or Online.
TOP can be administered on paper or completed online. A key technological innovation that reduces staff involvement and dramatically increases patient compliance is our patented in-home technology that administers, tracks and reminds patients to complete the TOP at home.
Reports are available instantly when a TOP is completed online and received within 15 minutes when submitted via fax.
