The TOP is written at a fifth grade reading level and takes an average of 8 minutes to complete. For adults and adolescents, there are 58 questions. For children, there are 48 questions.
Open Communication
With TOP, patients are more likely to disclose information that may be too stressful to share in conversation. TOP provides an efficient way for patients to share their full story. TOP is also an effective means to show patients their progress during treatment.
Convenient mobile, online administration
Our patented technology reminds, administers and automatically sends results to providers or EHR systems. Reminders are sent automatically based on appointment dates; the TOP can be completed on any device at the patient’s convenience. Staff involvement is minimized and patient compliance is dramatically increased.
Real-Time Feedback
Reports are available to providers instantly when a TOP is completed online. If a paper TOP is administered, results are sent within 15 minutes when submitted by fax.
