TOP Assessment

Outcome Referrals provides clinicians with a complete, multi-dimensional assessment that clearly illustrates a patient’s strengths, needs and progress during treatment. Because individual domain scores are measured against a US population norm, the level of pathology is easily tracked. We give clinicians the information they need to provide better behavioral healthcare.

Questionnaire
The Treatment Outcome Package (TOP) is a questionnaire that is filled out by patients before, during and after treatment. There are different questionnaires for children, adolescents and adults.
Quick and Easy

The TOP is written at a fifth grade reading level and takes an average of 8 minutes to complete. For adults and adolescents, there are 58 questions. For children, there are 48 questions.

Open Communication

With TOP, patients are more likely to disclose information that may be too stressful to share in conversation. TOP provides an efficient way for patients to share their full story. TOP is also an effective means to show patients their progress during treatment.

Convenient mobile, online administration

Our patented technology reminds, administers and automatically sends results to providers or EHR systems. Reminders are sent automatically based on appointment dates; the TOP can be completed on any device at the patient’s convenience. Staff involvement is minimized and patient compliance is dramatically increased.

Real-Time Feedback

Reports are available to providers instantly when a TOP is completed online. If a paper TOP is administered, results are sent within 15 minutes when submitted by fax.

Safe, Secure and HIPAA Compliant

At Outcome Referrals we ensure the highest standards of patient confidentiality, data security and complete HIPAA compliance.

Multi-dimensional Insights

At Outcome Referrals we measure a wide range of key symptom and functional domains and then quantify each factor to assess a patient’s well-being. These domains have been derived empirically, confirmed in large factor analytic studies and proven to be extraordinarily useful to clinicians.
